APPLICATION

The Golden Ribbon Foundation provides grants up to $1,000 to any eligible resident of Lane County, Oregon, who has been diagnosed with gynecologic cancer and is under a physician’s care for the disease. These grants provide complementary health treatments which may include:

  • Massage
  • Acupuncture
  • Reiki/Healing Touch/Energy Therapy
  • Nutritional Counseling
  • Movement Therapy: Yoga & Tai Chi
  • Mind/Body Workshop: breathing techniques, meditation, guided imagery, self hypnosis/biofeedback, movement, dancing, writing and drawing

Availability and types of treatment are subject to change without notice.

OUR SIMPLE APPLICATION PROCEDURE

To apply for a Golden Ribbon Foundation grant, please complete the following:

  • 2 page application form
  • 1 page oncologist referral form
  • 1 page HIPPA (Authorization for Release of Health Information)
  • 1 page Informed Consent & Acknowledgement of Risk form
